CME Certificates and Attendance Verification Certificates
The University of Texas MD Anderson Cancer Center designates this educational activity for a maximum of 2.25 AMA PRA Category 1 Credits(TM). Physicians should only claim credit commensurate with the extent of their participation in the activity.

Certificates awarding AMA PRA Category 1 Credits(TM) or certificates documenting attendance will be distributed to participants when an individual departs the conference.  To obtain a CME certificate, physicians must submit a completed evaluation questionnaire and a CME Verification Form.
 
Upon request, a record of attendance (certificate) will be provided on-site to other health care professionals for requesting credits in accordance with state nursing boards, specialty societies, or other professional associations.  
 
The University of Texas M. D. Anderson Cancer Center has implemented a process whereby everyone who is in a position to control the content of an educational activity must disclose all relevant financial relationships with any commercial interest that could potentially affect the information presented. M. D. Anderson also requires that all faculty disclose any unlabeled use or investigational use (not yet approved for any purpose) of pharmaceutical and medical device products. Specific disclosure will be made to the participants prior to the educational activity.

Attendee Refund/Cancellation Policy

Refunds are limited to meeting fees paid, and do not include any travel, lodging, transportation, or other fees paid independently of meeting registration at the time of refund request.  Meeting fees paid include, but are not limited to: event registration, workshop registration, and handout books.  Cancellations must be submitted to the AVA registration department ( ) in writing. 

To qualify for a full refund minus a $50 processing fee, written cancellation must be received by the AVA 2010 Scientific Meeting registration manager no later than 11:59 PM EST on August 26, 2010.  If initial payment included a membership fee, that portion will not be included in the refund.  There will be no refunds for cancellations received after August 26, 2010. 

Substitutions are encouraged and should be submitted to  .  Please include the original attendee’s name and the substitute’s full contact information.  No-shows are non-refundable.  Onsite purchases are non-refundable.
